Prisma Health

A pilot program within the Accountable Communities department of Prisma Health. We operate in the Upstate and the Midlands. We recruit and train volunteers to help those in our community who do not have a support system to help with practical needs at home for the chronically ill.

Our program is designed for chronically ill patients who need extra non-medical help when they go home. For example, our volunteers help with housework, yard work, deliver meals, visits, phone calls, transportation to dr. appts., etc... We also provide a sense of community and connection when so many feel isolated.
